In-School Immunization Program Clinic Reminder

NEXT CLINIC DATE: Thursday 4/27/2023

 

• One signed consent form allows your child to

participate in any In-School Immunization

Program (ISIP) clinic during the current school

year.

• If you have signed and returned a consent

form, your child will receive any vaccines they

are eligible for the day of the clinic, including

flu and COVID-19, unless any specific

vaccines were declined on the consent form.

• Please make sure the school nurse has your

child’s most up-to-date immunization records.

• Please make sure your child has eaten

breakfast before coming to school on the clinic

date.

• If you have a signed consent form, but no

longer want to participate in the

immunization program, please call us at
