Medication Pick-Up Reminder

If your student does not have medication at school please disregard this notice.

June 1, 2022

Dear Farrell B. Howell ECE-8 School Parents/Guardians:

It is that time of year again and all medications must go! If your student has medication at school, please make arrangements with the school nurse to pick up all medications/medical equipment that was brought to the health office to support your child’s health during the school year. All medications must be picked up by an adult and are not permitted to be sent home with your child.

If your child is participating in a summer program: Medication will NOT automatically be transferred to the summer school program.  All medication must be picked up by parent/guardian and given to the site on the first day of the summer program.  

MEDICATION NOT PICKED UP BY JUNE 3RD AT 1:00PM WILL BE WILL BE DISCARDED

The 2022-23 School year Denver Public Schools (DPS) Student Medication Agreement form is attached.

This form must be completed and signed by a medical provider and signed by parent/guardian. Just a reminder that all prescription medications must be in the original pharmacy labeled container and the label must match the medical provider form. All over-the-counter medications must be in the original package, unopened and labeled with the student’s name, and also match the medical provider form.
